A Service Request can be made to process the following:
- Address/Name/Photo Change
NOTE: 926B Active card holders are not required to have an up-to-date address as long as they are actively serving. While actively serving, card holders may use their ‘Home of Record’. 926C Retired/Separated card holders must always have a current address reflected on their card.
- Replace Lost/Stolen Card
- Change Card Category
NOTE: All requests to change card category must include a new application and applicable supporting documentation. All original cards will be marked ‘Invalid’ upon issuance of a new card.
- Card Renewal
NOTE: Renewals for 926B cards must include a new commander’s endorsement letter. 926C card holders must submit a new FBI background check, however, 926C Separated card holders that are still actively serving may submit a new commander’s endorsement letter in lieu of the background check.NOTE: If the current card expires within one year, customers will be required to pay the full renewal fee.
