Who Qualifies for a LEOSA Card?

926B must send to DCS:
  • Completed DCS LEOSA application, signed, dated and notarized
  • Commanding Officer Letter of Endorsement, signed and dated
  • Copy of Driver’s License or Government ID with photo (CACs are NOT ACCEPTED)
  • Passport Style Photo

926B Cost:
  • $145 via PayPal
  • $140 via Cashier’s Check or Money Order (NO PERSONAL CHECKS)

926C Retired/Separated
  • Separated means two things:
    • 1. Still actively serving but no longer in a Law Enforcement Career Field (i.e. Re-Classed), but after serving 10 years as a LEO
    • 2. Separated from military service, but after serving 10 years as a LEO

926C Retired/Separated, Service-Connected Medically Discharged
  • NO TIME REQUIREMENT
  • Medical Clearance letter required from DOD affiliated physician (i.e. PCM, VA physician, civilian physician that accepts Tricare)

926C must send in to DCS:
  • Completed DCS LEOSA application, signed, dated and notarized
  • Copy of Driver’s License or Government ID with photo (CACs are NOT ACCEPTED)
  • Passport Style Photo
  • FBI Identity History Summary
  • DD Form 214 (Member Copy 2 or 4), SF 50s, NGB 22s

Note: 926C Separated but Still Actively Serving (not in LE) applicants must also provide proof of 10 years in a Law Enforcement capacity
Note: 926C Separated but Still Actively Serving applicants may also submit a Commanding Officer Letter of Endorsement in lieu of the FBI Identity History Summary

926C Cost:
